I accept all major credit cards, as well as HSA and FSA cards. I use Stripe on the platform SimplePractice to charge all credit and debit cards.

I am currently in network with: Aetna, First Choice, First Health, Moda, and Pacificsource. I will be in network with Cigna/Evernorth through June 30, 2024.

If your insurance is not listed above therapy may still be covered by insurance as an out of network benefit. Out of network coverage varies widely. I encourage you to contact your insurance company about out-of-network benefits (including reimbursement, deductible, and number of sessions covered) prior to our first meeting. I can provide you with receipts (also called super-bills) to submit to your insurance company for reimbursement. You are responsible for the full session fee at the time of service.

I recommend calling your insurance company and asking the following questions:

-Is Dr. Sarah Nelson, Ph.D. with Rose and Fern Counseling at 3439 SE Hawthorne Blvd Portland Oregon 97214 a preferred/in network provider?

-What is my copay for each session?

-What is my annual deductible and how much have I spent towards my deductible so far?

-If Dr. Sarah Nelson is not in network with my insurance, what percent will you cover per session? What percentage is my responsibility?

-Are there any limits on how many sessions I can have with Dr. Sarah Nelson?

Current session rates range from $225 to $275 depending on the service provided.

If you are unable to make it to your scheduled appointment, please let me know as soon as possible. The first cancellation or no show with less than 24 hours notice results in a $100 fee while subsequent late cancellations and/or no shows will be billed at the full session rate. Please note that insurance does not reimburse or cover fees for late canceled appointments.
